The form provided below is valid for use as long as the following requirements are met:

• Printing:
  The form must be printed out in the same format.
• Language:
  English.
• Color:
  Black and red ink on white paper, or simply red ink on white paper.
• Size:
  Standard ISO A3: 297 x 420 mm. (11 3/4 in.) x (16 1/2 in.)
  Standard ISO A4: 297 x 210 mm. (11 3/4 in.) x (8 1/4 in.)
• North American equivalent:
  

  Legal 280 x 430 mm. (11 x 17 in.)
  Letter-size 280 x 215 mm. (11x 8 1/2 in.)

• Copies:
  Two (2) copies must be filled out and signed. One of the forms will remain with LAN CARGO and one will be affixed to the shipment.
